1. Complete and Submit Intake Forms
Begin by filling out and submitting our online intake forms.
2. Initial Contact
Within 48 hours of receiving your submission, a member of our team will reach out to discuss your participant's interest in the program.
3. Needs & Goals Assessment
We'll take time to learn more about your participant's needs, goals, and interests to ensure we can provide the individualized support required for a positive and successful experience.
4. Eligibility & Next Steps
If all eligibility requirements are met, we will provide a digital engagement contract and begin the registration process.
5. Program Start Date
Once the contract is finalized, a personalized start date will be scheduled.
