Intake Process

1. Complete and submit online intake forms.

2. You will then be contacted within 24 hours to discuss your participants interest in the program. 

3. We will schedule an in person meeting to provide your participant the opportunity to meet with our program staff. This will help support their transition into the program. Your participants needs, goals and interests will be discussed to ensure we can provide them with the individualized support they need to be successful. 

4. If all eligibility requirements are met, we will provide a digital engagement contract and start date will be determined.
